The PHQ-2 (Patient Health Questionnaire - 2) is an initial screening tool utilized in many mental health and primary care settings, to inquire about and assess the frequency of depressed mood, with a score ranging from 0 to 6. Individuals self-report on two questions, and this screen is deemed a "first-step" approach; individuals who screen positive require further evaluation to determine whether criteria for depressive disorder have been met.

Through my own practice, I've noticed that many clients feel intimidated by the stark, assessment-like aesthetic of the various forms I'm continually handing them, so I've designed these PHQ-2 handouts as a means of making the assessment process more welcoming to the client. These designs maintain the specifics of the original published assessments, however appear with different graphics, to cater to a wider range of clients' needs/understandings.
