The PHQ-9 is a multipurpose tool utilized in many mental health and primary care settings for screening, diagnosing, monitoring, and measuring the severity of depression. This module is comprised of 9 questions according to DSM criteria and allows patients to self-report on a rating scale of "0" (not at all) to "3" (nearly every day). This is a widely accepted screening tool that many clinicians have integrated into their weekly practice, in order to more effectively assess client symptoms, and to gain a more comprehensive understanding of how the individual client is affected by their depression symptoms.

Through my own practice, I've noticed that many clients feel intimidated by the stark, assessment-like aesthetic of the various forms I'm continually handing them, so I've designed these GAD-7 handouts as a means of making the assessment process more welcoming to the client. These designs maintain the specifics of the original published assessments, however appear with different graphics, to cater to a wider range of clients' needs/understandings.
