This download contains a wide variety of behavioral notifications and serves as a great communication tool when students make a poor choice. There are several different styles to choose from including both full or 1/2 sheets and different clip art styles.

Included is a reflections sheet that allows students to communicate what happened, why it happened, and what they will do differently next time.
All versions require a parent signature with a statement that reads:
"Thank you for taking time to discuss the importance of making good choices with your student. Please sign and return form on the following school day. I am confident tomorrow will be a better day!"
JUST ADDED: an entire set of notices that read "with your CHILD" in place of "student" for those choosing/needing that wording.
