This BITING Report Printable Form PDF prints 2 notices per page (one for the parent of the child who was bitten and the other for the parent of the child who was the biter). The biting report form is designed for licensed childcare centers, in-home daycares, nannies, and preschools. The download includes 6 pdf files (1 set of 3 each for color and black and white). You receive a print as-is form to fill out by hand, a fillable form to type in the information before you print, and, a fillable form with an editable biting policy field.

Biting is very common in early childhood and often through preschool. Children may bite for a variety of reasons. For example, babies and toddlers use their mouths to explore new toys or objects and may bite a person to see if they get a reaction. Biting can also be a way to try to get attention or used as a way to express emotions or feelings. Anger, fear, and frustration are strong emotions. Usually, young children lack the language skills to communicate how they feel. As a result, they may bite as a way of saying, "I don't like that!", "Pay attention to me!" or, "I was playing with that."

It is essential to communicate with the parents of the children in your care when a biting incident happens. While upsetting, a biting incident can happen for a variety of reasons. Therefore, all parties involved need to be informed to help identify what is causing the behavior. That way, they can be a part of helping the child cope with the feelings they have that caused them to bite in the first place.

Some of the reasons a child may bite include:

● Experiencing stress: new situations or circumstances

● Overstimulated: they do not know how to behave or get relief from too many feelings or emotions at one time

● Retaliating because they feel threatened: they think someone is being mean to them

● Feeling powerless: they use biting as a way to take control of the situation

● Seeking attention: any attention, even negative, is attention

This Bite Report form makes communicating with both sets of parents easier. The top portion is for the family of the bitten child. It has places for the date, time, bite location, treatment applied, additional comments, and signature lines for the caregiver and parent. It also has a statement regarding biting at the bottom to help reassure the parent that while upsetting, biting is normal and what your policy is regarding how you handle the situation.

The bottom portion of the form is for the parent of the child who was the biter. It has spaces for the date, time, location of the bite, where it happened, the activity at the time of the incident, corrective action taken by you, additional comments, and signature lines for the caregiver and the parent. There is a statement at the bottom to communicate about biting and suggest further discussion as to coordinating strategies for both your facility and the child's home to help the child deal with what is causing them to bite.

You can edit the bottom biting policy on both sections of the form to match your facility's policy on handling a biting incident.
