Culinary Arts Application and Teacher Recommendation Form- Editable

Our Culinary Arts Program became so popular that I had to create an application process!

This is the student application and the teacher recommendation form we use. The student rates their strengths and weaknesses and tells you why they want to be in the class while the recommending teacher or counselor tells you all about what type of student and individual they really are. The students get so excited when they find out they made it into the program! This avoids the "dumping ground" scenario some school systems still use.

When you get kids in your class that actually WANT to be there, you have fewer discipline problems and they will work hard to stay in the program!
