Description

This is a daily behavioral point sheet I use in the middle school setting to help students manage their behavioral goals.

Students carry it in a folder and the teacher in each bell completes it and initials- I total their points at the end of each day. Students earn a maximum of 3 points per objective per bell and have a daily percentage goal to meet to earn a reward. This works well with the ED Level System; I also use it with LD, AUT, and OHI students.

It is easy to add or remove goals and bells to customize this for each student- I included several samples with goals prefilled. I like adding the homework box to ensure the parent gets a copy. The point sheet makes it easy to collect data for IEP goals and allows daily 2-way communication with parents.
If you change the number of goals or bells, simply multiply 3 points times # of goals times # of bells to calculate total number of points possible.
