Google Sheets file with Documentation Template for Caseload Organization, Direct Session Notes, and Consult Notes. File starts with a caseload roster template that allows you to divide student by classroom, school, direct v consult whatever works best for your caseload. I currently have one file for each school I work at. The caseload roster allows you to input the student, the current service level, their annual IEP date, and any important contacts for that student. Highlight upcoming IEPs to help yourself see what needs to be prioritized based on due dates.
Next there are tabs you can hit "duplicate" to create a tab for each student. There is a direct template (for individual and groups) or a consult template. Simply fill out each of the information sections for a way to organize your student's goals and therapy notes. There are also clickable options for areas of need addressed to speed up note writing (abbreviations for fine motor, bilateral coordination, visual perceptual, upper extremity, gross motor, motor planning, postural stability, sensory). At the end of the year you have a printable file with the students name, goal, school year, and session notes for you to put into each students file.
(8/7/2022) An additional more detailed template option has also been added at the request of buyers whose districts are seeking more detailed notes. This template includes everything above but allows you to type time in/out, check box for direct/indirect, check box options for location service, and billable vs. non-billable minutes provided.
The file is a google editable so any location or area of need box can be changed and copy pasted into the other boxes if needed to better suit your caseload's needs.

Google sheets save automatically as you work. This file helps prevent lost notes due to messy paper, technical issues, or a laptop running out of battery!
