This Word Document consists of a parent teacher contact form (template) that you can use to take notes during or immediately after a meeting, phone conference or email message. It is easy to forget important items discussed during parent conferences held on back to school night, or right before grades are due. This is a great template that will make it easy to recall important information. I have used this form as one of my end of year artifacts for my final (summative) teacher evaluation meeting and the principal loved it. Perfect evidence for Domain IV!!

The template is editable based on your needs and it is applicable to almost all subjects and grades. There is a second page to take additional notes so you can print double sided.

Just print enough copies for your class, maybe a few more just in case, and keep them in a binder for your records. The same template can be used for writing notes about any follow up contacts that you make.
