How to use this Student Information Survey:
This editable student survey can be used as a digital back-to-school activity or for paper-based learning. Promote equity, growth mindset, and class community in any subject area using the sample question prompts provided with this resource.
These get to know your student question are broken up into four sections: an introduction to the student, their family, their interests, and other important information:
- The first section will prompt the student to provide information about their identity.
- The second section will prompt the student to share about their family and cultural community.
- The third section will inquire about interests and responsibilities.
- The final section will address health-related concerns, and offer the student the opportunity to share any additional information.
To promote a growth mindset, this student information sheet also prompts students to consider their goals and anticipated challenges for the school year.
In addition to collecting important information from your students, you can also share a bit about yourself at the beginning of this survey to model the responses they may offer and to be personable on your own behalf.
All of these student survey questions are editable. Be sure to also edit the blurb about you! You can also edit the survey to cater to your subject area. If you do not wish to edit the survey, there is a ready-to-print student information sheet provided.

Great form with important details to know about your students to get to know them and their families. I teach in a school that serves a diverse student body online, so having questions about languages spoken at home and parent communication is key. I edited a few that I didn't need for my groups, but would say I used 95% of the original form :) Very pleased.
