Description
Having food permission from parents can give you a little more peace of mind. With so many allergies impacting kids every year it is important to have additional forms that address the issue of food. Throughout the school year I like to incorporate students trying new foods--this form comes in handy BIG time. I send it home during the first week of school but it truly can be incorporated at any time.
There are two versions of this form. One can be copied on to bright colored paper as the clip art included is black and white. The other can be printed for direct use with colored clip art.
