(I originally created this Teacher Input Questionnaire in 2014. Since then, I have included a Google Form and I have been tweaking it for my classroom use and I am finally uploading the updated form for all of you!)
Preparing for an IEP report or any other evaluation report can be quite stressful and incredibly time consuming. Although I can’t make it easier for you – trust me I wish I could :) , I can at least provide you with a little bit of help when having to ask for teacher input!
In my first few years of teaching, I realized that asking for input from all of my students’ teachers was at times difficult. Sometimes I would get too much information, and other times I wouldn’t get enough at all – basically because some teachers saw my students more than others. I did, however, want them to at least provide the same general information for the parents to see, while also adding their own additional comments. So, I “prettied up” my original Teacher Input Questionnaire and now want to share it with all of you!

Teachers are able to provide input for student strengths and weaknesses, student performance, student behavior, and teacher adaptations. Some parts of the questionnaire require the teacher to simply circle appropriate levels, while other parts allow the teacher to write any additional comments.
If you are new to using Google Forms I have provided easy to follow Google Form instructions. The time is takes to create and set-up a Google Form (and it's collection spreadsheet) I have done for you! ...because let's be real, as teachers we need all the extra time we can get.
