Have your students fill this out weekly, monthly, quarterly, yearly, or by semester to reflect on each part of the year! I love sharing this form with families at conferences as a great ice breaker. Also works great for student led conferences!
This form asks students to reflect on their learning, favorite and least favorite parts of their grade, and areas for growth. An evaluation of behavioral skills is also listed for students to rate themselves by a score of 1-5. This template is editable so you can edit the questions and prompts to fit your students needs!
