Speech Therapy Report Template - OWLS II

This template is for the Oral and Written Language Scales, (OWLS II) for children and young adults aged 3 through 21 years. There are two subtests included, Listening Comprehension and Oral Expression. This template can be inserted into the body of your report. The template is 4 pages long including the cover page.

** Full Disclosure- This report template was written by me, as an elementary school-based Speech-Language Pathologist. It can be easily adapted to other tests and specialties.
