Description

All of these files are either in Excel Spreadsheets or in Word and are easy to edit for your needs. A few are examples and have my information in them.These are the forms that I have included in this file.

Attendance Sheet- This is what I use for rehearsals to keep track of who is at rehearsals and who isn't.

Audition Sign Up- I have the kids sign up in blocks of 6 kids for each 15 minutes. This makes sure that I know who is supposed to there when and it also shows how well the kids do with commitment.

Cast Member Contract- This is the sample Cast and Crew contract that I use for my shows.

Conflict Schedule- This is something I have the kids fill out so I know what they schedule conflicts are.

Contact Sheet- A sample contact sheet for shows.

Costume Cue Sheet- This is to help the stage managers keep the costume changes in order.

Costume Hangers- This is what I put on the hangers of every students costume.

Crew Audition Packet- This is a sample crew audition packet.

Demerit Contract- This is the demerit contract that I use for every production.

Lighting Cue- This is to help the stage managers keep the lighting cues in order.

Music Log- This is a music log that I use for UIL One Act Play.

Props Catalog- I use this when I make a list of all the props that I will need for a show to figure out what I will need to buy, make, etc.

Props Cue Sheet- This is to help the stage managers keep the props in order.

Sample Character Breakdown- I use this to know who is in what scene. I also use this sheet to let the kids know which rehearsals they need to be at. Sometimes I only call certain scenes to rehearsals.

Sound Cue Sheet- This is to help the stage managers keep the sound cues in order.

TShirt Order Form- I use this template and change it with every production I do.
