Description

This is a modified worksheet from the middle school number relationships/computation curriculum. It is excellent for special education students who require an alternate or modified curriculum.
Five fractions are given in a chart. There is a bank of options to choose from to fill in the rest of the chart. Students must fill in the equivalent forms for the fractions in decimals and percentages from the bank.
This worksheet aligns with Maryland content standard:
Given fractions, decimals, and percents in written form, student will identify the equivalent forms.
There is a helpful key in the top right corner to assist with IEP data for students with special needs. The key makes documentation easy, including the score, prompts used, and whether or not the objective has been mastered.
