Allergy Awareness Week Unit

Office of School Nurse Stickers for Parent Communication
These stickers will assist you in mass communication with parents. Labels will print on any normal sized blank address labels.

Classroom Pass System
Keeping up with where students are going and how many times they go during a grading period can be a tedious task. For me, having a sign-out sheet only partially worked because students always managed to forget some part of the required info. I created these labels to give a strip out to each student at the beginning of each grading period. It could be printed on regular paper and torn into tabs or it can be printed on labels that students peel off to apply to a paper at the door as they leave.

Nurse Communication Notes
Often it is difficult to understand why a student was sent to you, especially if they are very young. This note will help you communicate better with the teachers. It is editable in Microsoft Word, and can be printed as 5x7 so that you can have two to a page if desired.

Phonemic Awareness: Identifying Phonemic Errors in the Medial Position
Phonemic Awareness Practice:This worksheet targets listening skills and can be helpful for auditory training practice for students with hearing aids or cochlear implants. The listener will identify where the phonemic error is in the sentence and correct the sentence so that it makes sense.

Documentation is key! Here is a handy form I have used for years to help me keep track of who I have spoken with and why! I love to print out the blank form and keep them in a binder so I can fill them out quickly and keep my day moving.

Monthly/Quarterly Contact Log
A monthly or quarterly contact log that I use for my IEP caseload. It gives a spot for the students name and then boxes for a significant amount of summarization from each contact you have with the students.

Classroom Sign Out/In Sheet
If my students need to leave my room, they must first sign out to go to the restroom or whatever their destination is. When they return to my classroom, they must sign back in. This is very helpful in accountability.
