For the most part, they allow us to present ourselves in a positive light, and help us demonstrate respect — for other people as well as for ourselves — which is something we need in this world more than ever. This c

Students fill out the slip with the CHAMPS expectations. Then cut the slip on the dotted line. The student takes home the CHAMPS part for parent communication. The teacher keeps the small portion in the child's file. The small portions can be used to collect behavior data on individual children, or can be tallied onto a classr

School Counselor Self Referral Student Passes

At the beginning of each year, I laminate one of each of these passes for each teacher in my building. If a student is in crisis or has an emergency they can use the "I need to see the counselor NOW" pass (with red light) ON RED PAPER, but if not, they use the "I need to fill out a counselor request slip" (with yellow light) ON YELLOW PAPER and can return to class. This helps me prioritize and triage as necessary and helps hall monitors, admin, and clerks route the student in the correct place.
